Registration form (console)

The US launch registration uses an extended form. It includes all fields from the standard country launch registration, plus additional sections for Company information (company ID or EIN, company name, DBA name, company website, legal entity type, stock symbol and exchange if publicly traded, company overview, and industry sector), a Company address (address lines, city, state/province, postal code, and country code), Extended contact information (a primary contact phone number in addition to the baseline contact details), and RCS partner information (RCS partner name, contact name, and contact email).
