The United Kingdom launch registration uses a custom form that collects brand information, use case selection, your registered company name as it appears on Companies House, your industry, your registered company address, your Companies House registration number, a screen recording, and privacy policy and terms of service URLs.

**Note**  
Your company name and address must exactly match your Companies House registration. Discrepancies will cause delays in the approval process.
